Recently a number of studies have been done in Type I Diabetes of which two have had an outstanding impact in giving further insight into the management of Type I Diabetes. The DCCT study showed that good glycemic control was possible and with good control of Type I diabetes, we could reduce the incidence of eye damage by 76% nerve damage in the feet by 60%, and kidney damage by 56%. This study showed that good control of blood sugar was very important and protected against various organ damage in the body.
The Captopril study in Type I Diabetes was another excellent example that showed that captopril when used in diabetic patients not only reduced the blood pressure, but protects the kidneys from damage and worsening of kidney failure.
The recent arrival of new class for agent for blood pressure control called Angiotensin II Receptor Blockers is an excellent addition to the choices we have in blood pressure control. There are about a dozen drugs in this class that are being studied but Losartan is the only one in this class approved by FDA for High Blood Pressure.
As 90-95% of the diabetics have Type II diabetes, it was only natural that major studies would be undertaken in this field to prevent the complications, in particular, the kidney damage and cardiovascular complications. There are a number of larger studies going on to address these issue in Type II Diabetes.
The Diabetic & Hypertension Center has embarked on a new study called RENAAL study, which is a multi center international study with Losartan in Type II Diabetes with kidney damage. The study will be for five years in patients between ages 31 and 70 with mild kidney failure and protein leaks.
The basic entry criteria are:
- Type II Diabetes diagnosed after age 30
- Age 31-70
- Creatinine of 1.5-3.0 mg/dL(all women and any men<60kg. May have a lower limit of 1.3)
- screening urine protein of > 1+by dipstick
- urine albumin/creatine ration >= 300 mg/g on first morning void
- normo or hypertensive
- glycoslyated HgbA1c < 12%
The goal of the study is:
- To compare conventional therapy with the research drug, Losartan, to see if it will impede the progress of renal disease, need for dialysis, or death
- To compare a conventional therapy and see if the research drug, Losartan, will decrease the cardiovascular complications or death.
There is no charge to the patient to participate in the study and they may benefit from close monitoring of their health over the next several years. Their medical management, high blood pressure medications, and investigations are also free.
If the patients have to travel to go to the study site, they will be compensated for the cost of travel.
This will be a landmark study and the results will have long-lasting effect on the way we manage Type II Diabetes.
